Plan includes:
- Two Cleanings
- Set of Annual X-rays
- Two Exams
- Two Fluoride Treatments
- 25% off Additional Treatment needs
* Orthodontic treatment excluded
* The plan runs from the date you purchase to the same date the following year (Ex: 05/01/22/ to 05/01/23). The second recare appointment must be within 8 months of your sign up date.*
* You have the right to cancel at anytime with a written request. We have 30 days to apply a refund. The refund will be based on a pro-rated reimbursement less a $40.00 fee
