We welcome new patients. Appointments can be scheduled through our online appointment request form or during office hours, either by phone/text at 614-443-4625 or in person at 79 Thurman Avenue, Columbus, Ohio, 43206. 

If you are unable to keep an appointment, we ask that you kindly give us at least a 48 hour notice by calling our office  at 614-443-4625.

This will allow us to contact other patients waiting for an appointment and give them an chance to be seen and treated sooner.

 A $50 fee may be charged for no-shows and cancellations made without sufficient notice. 

We accept cash, checks, Care Credit and all major credit cards. 

For cases requiring extensive treatment, we will be happy to work with you to make everything as easy and affordable as possible. 

We accept most major insurance plans as we are a PPO provider. 

We are not in network with any Medicaid or State funded insurances.

 If we do not carry your particular plan, you will be required to pay our full fee at the time of service, and we will be happy to submit a claim on your behalf with your insurance company.
